Some infection prevention problems can wait for the next committee meeting. These can't:

  • A sterilizer, washer-disinfector, or high-level disinfection failure, with instruments already used on patients

  • A CMS immediate jeopardy finding

  • An outbreak or cluster you can't yet explain

  • A patient or staff exposure that will require notification

  • Construction, flooding, or storm damage affecting patient care areas

  • A reprocessing or sterile storage breach discovered during a survey

CMS Immediate Jeopardy Response

An immediate jeopardy finding starts a clock. You need a removal plan that satisfies the surveyor quickly, and a corrective action plan that holds up when they come back to verify it. Those are two different pieces of work, and facilities under pressure often produce a removal plan that buys a few days and creates problems later.

We've worked with facilities through IJ findings tied to infection prevention and patient care policies and processes, sterile processing, high-level disinfection, isolation practice, and environmental cleaning.
